Application
for Air Contaminant Discharge Permit
This "long" form is used for first time application, for modification of an
existing Air Contaminant Discharge Permit (ACDP) and for renewal of an existing
ACDP.
The two following documents may be useful in completing the "long"

for Administrative Amendment to Air Contaminant Discharge Permit
The "short" two-page form is used to report the following changes:
- facility or company name,
- facility or company address (street or mailing),
- ownership/parent company (name or address),
- facility contact person, and
- person authorized to receive permits and invoices.
This form is also used to notify LRAPA of changes to any production processes
or equipment.
Construction Application
This form is used by persons who are proposing to construct a new industry or
expand an existing industrian process.

Contact List for LUCS
This form is used to determine that LRAPA permits are consistent with the local
government comprehensive plan. A completed LUCS is required for:
- every new (first-time application),
- any currently-permitted facility that has not yet been asked for one, and,
- whenever there is any physical change or change of operation of and air
pollution source that results in a net significant emission rate increase
(as defined in Section 12-001 or Section 38-005(12) of LRAPA's rules.
Note: Portable air pollution sources and soil remediation
projects are not required to submit a LUCS.
Relocation Notice for Portable Plants
This is a reporting form for reporting relocation of a portable plant. Reporting relocation of a portable plant to Lane Regional Air Protection Agency (LRAPA) is a requirement of a LRAPA permit to operate in Lane County, Oregon. Please note that relocation reporting to Oregon Department of Environmental Quality does not fulfill the requirements of your LRAPA permit for operation in Lane County.
